What prints in block 32/33 on the HCFA form?

For all of the following, these options must be set before the order is created!

In System/Setup Company/System you will fill in the "Company" name and address information.

In System/Setup Company/System you will fill in the "Remit to" name and address information only if it is different from the "Company" information.

Block 33 will always print the "Company" name and address unless the "Remit to" address is filled in, in which case it will print the "Remit to" name and address.

In Receivables/Patient/Patient Information you will put in the "Ship to" address only if it is different from the patients address.

In System/Setup Company/Orders you will find the question "Push the Ship-to address into the Facility?" and answer either "Y" or "N".
